You must bring the following documentation to the office on the day of your consultation:

In addition you will need to provide these documents in order to initiate the insurance authorization process:

  • Letter from your primary care physician, on letterhead, stating that he/she recommends you for weight-loss surgery. This is not an insurance referral.

  • Records of a complete physical from your physician, including standard blood test (CBC, comprehensive metabolic, liver test, and TSH)

  • Documentation of at least six months' participation in a medically supervised weight-loss program

  • Medical history and physical from your primary care physician, including any co-morbidities (e.g., diabetes, high blood pressure)
